Countless books, tapes, videos, and articles have been published giving advice and philosophy on the secret to finding happiness and success in one’s life. To try to codify all those thousands of philosophies into a single article, would be an impossible task. However, there is one key element that is constantly emphasized, throughout all of these paths to happiness and well being. It goes beyond merely setting goals or eating correctly or dieting or religious faith. When you look at what all these mentors, philosophers, and gurus are teaching, they are basically focusing on the absolute need for you to adjust and focus your own thoughts. In other words, you are what you think.

Yes, I know…you’ve heard this all before, but before you turn away, there is a very vital point that you need to understand. Your thoughts, today, do NOT govern your present situation. In fact, your situation, condition, state of being or whatever you want to label it is the product of your PAST THOUGHTS. You are constantly creating new thoughts or reinforcing old ones, and it’s those thoughts that have shaped your present condition, and will also, invariably, create your future life conditions.

If you desire to change your condition, then you need to shift your focus of how you are thinking. This also requires a little fine tuning on your part. The truth is, our minds are amazing, and thoughts are the energy that steers us. Whatever it is we focus our thoughts on, is what we are setting our sights on. The strange, but logical thing about this is even when we are thinking about something we don’t want, since our attention is on that thing that we want to avoid, we tend to get more of it. A good axiom is that we tend to get what we resist the most.

For example: If you hold on to a thought that says: “I don’t want anymore bills,” your attention is on THE BILLS and that is what you will keep accumulating. However, if you shift your thought process a bit to the left and start thinking: “I have the money and means to live without worrying over bills,” your mind and thoughts now focus on having money rather than bills. Get it?

Another very important point to always remember is to put your goals in the PRESENT tense. If you keep thinking that you want to have (fill in the blank) or someday you will get (fill in the blank), it will always stay just out of your reach. Instead, focus your thoughts on already HAVING what it is you want, and let your mind’s energy do the rest.

Now I know this all sounds like wishful thinking, but the simple truth is that it works. How long will it take for your thoughts to materialize into the real world? I can’t give you an answer to that. However, if you want proof that this is valid, take a look at where you presently are, and give a good hard look at how you let your thoughts be, in your past. YOU CREATED YOUR PRESENT. YOU ALSO HAVE IT IN YOUR POWER TO RE-CREATE YOUR FUTURE.

Let me begin with a story. King Wu, the founder of the Zhou dynasty (1122-220BC) has just defeated Chou Hsin, the last emperor of the Shang dynasty(1700-1112BC) with the help of his strategist Jiang Ziya (Taigong Wang). One day, King Wu was at the countryside with his followers when they passed by the rice fields.

Strategist Jiang Ziya then asked the King to follow the poor peasant by ploughing the field for the poor man! ‘What? How can you ask the King to plough the field for such a lowly man?’ exclaimed all the generals. However, the King remained calm and respecting the wisdom of his beloved strategist, came down and set his hands to the plough. All the King ’s followers yelled and protested as the King, splattered with mud, panted through his task. The King was tired but he struggled on despite the generals urging him to stop.

After some time, the King was too tired to continue. Strategist Jiang asked the King, ‘Your Highness, how many steps have you taken?’ King Wu took a look and said,’ Nine hundred steps in all.’ Strategist Jiang then prophesied that the Zhou dynasty would last as many years as the number of steps King Wu has taken. Susprised and alarmed, the King wanted to return to the field and plough further so that his dynasty could last longer. But he was stopped by Jiang.’ It is destiny, my Highness.’

True enough, the Zhou dynasty went on to last over nine hundred years. It was infact one of the longest era in the dynastic history of China. King Zhou and his descendants continued to remain humble and treat their people with care and concern. The chinese word Shou Hsing which means longevity actually incoporated the sub-character of ‘plough’ according to this legend.

We can learn from this story. To be successful, it is not good enough to have wisdom and bravery. We need the power of benevolence for the people around us. Only when leaders care for their people or those below them that they will gain their support. The most powerful tool to weather any crisis is the unity born out of care and concern between leaders and followers!

EMS A-Z Series “R” - RSI and Ready Or Not By James Hoffman

If you have been reading the A-Z Series you know that I have been trying to avoid too much clinical type articles and have been focusing more on general ideas on what we face each day as EMS professionals and also why the basics of what we learn in the beginning of our careers applies to what we do every day.

When I mention RSI or Rapid Sequence Intubation I don’t want to get into the clinical aspect of it on why Succinylcholine over Vecuronium or on Mallampati classes. There is a wealth of clinical information on this on sites like EMS1.com and JEMS.com to name a few.

What I would like to mention is that RSI is becoming more of a standard in many EMS agencies and most have a specific training requirement of their paramedics to be able to perform this skill in the field. RSI encompasses many dangers and difficulties for the provider and it truly brings us as EMS professionals to the next level of care and skill level.

When many are suggesting that endotracheal intubation in the pre hospital setting should be a thing of the past. The fact that RSI is such a hot topic, I think strengthens our need for advanced airway management in the field. The key is having the proper training not only for the skill itself, but also understanding the medications and their properties. That is where the danger lies and some providers may take this danger too lightly. Which of course can only bring us back to the thought process of sending ETI to the bench.

We as providers must take ETI and in turn RSI seriously. That means getting the proper training, seeking out clinical opportunities to perform the task and most of all knowing when it just isn’t possible and not letting our ego get in the way of what is best for the patient.

While I do see the advantages to non visual airway management. There will always be a place for true advanced airway techniques. After all “airway” is first on the list. Without being able to secure and maintain it - everything else is just spinning our wheels.

I don’t pretend to have all the answers or know it all when it comes to this subject. You can talk to ten people and get ten different views. The one view though I think we can all agree upon is that education, training and practice will allow us to continue providing true pre hospital care that includes everything the ABC’s encompass. Instead of focusing on just the pre hospital transport part.

Along the lines of all this training and advances in how we provide care. You must be ready to do so. When I say ready or not, I mean just that. You are either ready or you are not ready. So, ready for what? Well, think about that for a moment. Ready can relate to checking your ambulance to make sure it not only runs and has the lights working, but also that you have all your equipment and that it works as well. From your laryngyscope light bulb turning on to having enough non rebreather oxygen masks.

It can also apply to being ready as a professional and knowing your local protocols, procedures and how to apply them. Being ready to read the 12 lead ECG, start the IV or just plain lift the patient.

I think you get the idea I am going for here. Be the EMS professional that is ready. Ready in training, equipment, knowledge and attitude. The patients may not say it, but ready or not here they come.
