You probably heard the story of the Zen master who showed his students a jar full of sand and assorted sized rocks and asked them how to add a few larger rocks to the jar without throwing out any of its contents.

They of course answered that it couldn’t be done; there was no more room.

Isn’t that how you feel about your latest personal change project?

There is no more room, so no wonder part of you sabotages the personal improvement or profit making project that you say you want to institute.

Your mind is full, your time is taken, and part of you rebels. No matter how important a big rock is, it can’t go into your jar.

Yes it can

Your jar can hold more than it appears; the trick is to put the bigger rocks in first, then the smaller and smaller ones, finishing with the sand. There is room for everything because the smaller particles just filter through whatever small space is available.

Try it for yourself one time; seeing it is a lot more powerful than reading about it.

What are your big rocks?

Mine this week was starting to exercise daily (I know, so predictable a New Year’s resolution), and I followed the typical pattern when it comes to change:

Day 1: Did great

Day 2: Didn’t do any. I got busy and told myself I’d do it later when I wasn’t as mentally productive, and then ran out of willingness later in the day. How embarrassing since I teach people how to make changes in their lives!

Why didn’t I put my big rock in first? I didn’t think I needed to; I thought for sure I would exercise later. After all, until recently I was used to frequent workouts and didn’t have to struggle to do it. I certainly didn’t feel any internal resistance and was looking forward to it.

But in the last 4 months, I have been spending most of my time sitting in front of the computer and let my gym membership lapse (sound familiar?). I had left myself get out of the exercise habit, at an age where it takes a little more effort to get my body motivated.

Day 3: I put the rock in first and started the day with exercise.

Now putting the rock in first doesn’t necessarily mean that you have to do it first thing in the morning. It just happens to be a good idea for many people especially when it comes to exercising. Your past behavior will tell you what you need to do to make your big rock a priority before other things.

The most frequent strategic mistake

I still find myself going down that road; see if it’s familiar thinking: “I’ll get all the small easy stuff out of the way to clear the decks, and then I’ll be freer to get started on my priority.”

So I’ll check e-mails…and still be piddling about an hour later. Twitter and Facebook are two other great ones to procrastinate with.

Yes, e-mails and Social Media may be essential to your business, but they are quite distracting and can be done in small increments. They are the sand rather than the big rock.

Big Rocks are usually either:

Changes of habits
Things we might resist but need to do
Goals that might bring up fear of failure or rejection
Creative projects needing concentration
And requiring sustained effort
Clearing the decks first opens the door to your internal resistance distracting you from ever starting on the more important, and scarier, stuff.

EMS A-Z Series “R” - RSI and Ready Or Not By James Hoffman

If you have been reading the A-Z Series you know that I have been trying to avoid too much clinical type articles and have been focusing more on general ideas on what we face each day as EMS professionals and also why the basics of what we learn in the beginning of our careers applies to what we do every day.

When I mention RSI or Rapid Sequence Intubation I don’t want to get into the clinical aspect of it on why Succinylcholine over Vecuronium or on Mallampati classes. There is a wealth of clinical information on this on sites like EMS1.com and JEMS.com to name a few.

What I would like to mention is that RSI is becoming more of a standard in many EMS agencies and most have a specific training requirement of their paramedics to be able to perform this skill in the field. RSI encompasses many dangers and difficulties for the provider and it truly brings us as EMS professionals to the next level of care and skill level.

When many are suggesting that endotracheal intubation in the pre hospital setting should be a thing of the past. The fact that RSI is such a hot topic, I think strengthens our need for advanced airway management in the field. The key is having the proper training not only for the skill itself, but also understanding the medications and their properties. That is where the danger lies and some providers may take this danger too lightly. Which of course can only bring us back to the thought process of sending ETI to the bench.

We as providers must take ETI and in turn RSI seriously. That means getting the proper training, seeking out clinical opportunities to perform the task and most of all knowing when it just isn’t possible and not letting our ego get in the way of what is best for the patient.

While I do see the advantages to non visual airway management. There will always be a place for true advanced airway techniques. After all “airway” is first on the list. Without being able to secure and maintain it - everything else is just spinning our wheels.

I don’t pretend to have all the answers or know it all when it comes to this subject. You can talk to ten people and get ten different views. The one view though I think we can all agree upon is that education, training and practice will allow us to continue providing true pre hospital care that includes everything the ABC’s encompass. Instead of focusing on just the pre hospital transport part.

Along the lines of all this training and advances in how we provide care. You must be ready to do so. When I say ready or not, I mean just that. You are either ready or you are not ready. So, ready for what? Well, think about that for a moment. Ready can relate to checking your ambulance to make sure it not only runs and has the lights working, but also that you have all your equipment and that it works as well. From your laryngyscope light bulb turning on to having enough non rebreather oxygen masks.

It can also apply to being ready as a professional and knowing your local protocols, procedures and how to apply them. Being ready to read the 12 lead ECG, start the IV or just plain lift the patient.

I think you get the idea I am going for here. Be the EMS professional that is ready. Ready in training, equipment, knowledge and attitude. The patients may not say it, but ready or not here they come.

I remember many years back how hard it was getting a good education, especially with all of the partying that was going on in the dorm rooms and how long it ook to find a parking spot at the university. These days you can even get a doctoral degree or a Masters degree online from a University that is highly accredited such as Keller Graduate School of Management which offered masters Degrees to student thats specialized in doing commerce over the internet, or the University of Nebraska at Kearney which offers very eclectic business oriented fields of knowledge ranging from international business to understanding very complex economic concepts. Just remember before signing up for any online educational course to do your due diligence on the college that is offering these courses to make sure that they are accredited and are valued by the more established land based universities as once you put in your time to get your online degree you cannot get that time or effort back.
